Student / Athletes needing a sports physical prior to their season need to have their medical provider complete the NYS SCHOOL HEALTH EXAMINATION FORM found below and on the  Athletics page.  Please complete and submit to the school at least three work days before the season starts.  This can be dropped off at the Jr.-Sr. High School Main Office or faxed to the Nurse's Office at 607-967-6345.
